実際 = real (実) + occasion (際): on the real occasion, i.e. in fact
実 means truth or reality (as in 事実, fact) and 際 means occasion or edge (as in 国際, international). Put together: when it comes to the real occasion, in actuality. The small っ doubles the s: jitsu + sai becomes jissai.
