📋 事 (matter/affair) + 项 (item) = a matter-item: an issue or agenda item
事 means affair or matter; 项 means item or entry (as on a list). Together they name a single point of business — an issue, topic, or agenda item, like 注意事项 (points for attention).
事 — hand 又 holding a banner/tablet on a pole
Originally a pictograph of a hand (又) grasping a pole or ceremonial banner used by an official, depicting the act of conducting court business. From this it came to mean 'affair, matter, business, service.' It shares an ancient origin with 史 (scribe) and 吏 (official).
项 — 工 (gōng, phonetic) + 頁/页 (head)
项 originally meant the nape or back of the neck, with 頁 (head) giving the meaning and 工 supplying the sound. Over time it broadened to mean 'item' or 'category,' as in 项目 (project/item), while the original neck sense survives in compounds like 颈项.
